WHAT SERVICES DOES HOSPICE PROVIDE?

HOW DOES HOSPICE CARE WORK?

Typically, a family member serves as the primary caregiver and, when appropriate, helps make decisions for the patient. Members of the Hospice staff make regular visits to assess the patient and provide additional care or other services. The Hospice team develops a care plan that meets each patient’s individual needs, as well as the needs of the family. The team consists of:

  1. The patient’s personal physician
  2. The Hospice physician
  3. Nurses
  4. Home health aides
  5. Social workers
  6. Clergy
  7. Dietitian
  8. Trained volunteers
  9. Speech, physical, and occupational therapists, if needed
