WHO IS ELIGIBLE?
Admission to Hospice of Warren County is open to any patient who
- Resides in the Warren County area
- Has his/her doctor’s and the Hospice Medical Director’s certification that he/she has a life expectancy of six months or less if the illness runs its normal course
- Has a family member or other appropriate person available to assist with care needs and decision-making
- Chooses to receive Hospice care rather than curative treatments for the illness
The admission process begins with a phone call to Hospice made by the patient, a family member, friend, clergy, physician, or other healthcare professional. The patient’s personal physician is then contacted to make sure the patient is eligible for Hospice services. A time will be arranged for Hospice staff to visit with the patient and family to explain how Hospice can help. If Hospice is the service you want, Hospice will take care of the rest.
